Evan Smoak a été enrôlé dès son plus jeune âge dans un programme secret consistant à former des orphelins aux techniques les plus efficaces d'élimination physique des ennemis de l'État lorsque le recours à la force n'est pas officiellement possible. Il a perdu son identité pour devenir « Orphelin X », mais il n'a pas perdu toute son humanité. Pendant des années, il a obéi aux ordres de son instructeur, il a agi en solitaire à la « neutralisation » de crapules et de terroristes.
